Designed for your baby’s precious first year, this plan includes the first in office newborn visit, 2 week visit, 1 month visit, 2 month visit, 4 month visit, 6 month visit, 9 month visit and 12 month visit. Plan includes necessary sick visits and telemedicine visits as well as all in-office labs and in-office treatments required for visits included in the plan. The plan requires a 1 year commitment at a rate of $300/month. All vaccines will be billed directly to patient’s insurance.
