This plan is designed for patient’s who prefer to pay for each service at the time of the service. This plan requires a $ 2000 retainer fee. All charges will be deducted from the retainer fee until exhausted for the year. Additional charges will be charged at the same rates below. At the end of each visit you may request documentation that you may submit for out of network benefits or HSA plan.

Sick visit $ 250

Telemedicine $ 250

Second Opinion $ 500

In house labs $ 50-200

The $ 2000 enrollment fee will secure your child’s spot and will be applied toward their first visit.
